Special ServicesClinical Case Management: 467-5857

Clinical Case Management offers Swing Bed (Skilled Nursing Care) when patients are not able to go home with home health care or don’t want to go to a long-term healthcare facility (nursing home). Swing Bed is the perfect alternative! Since GPCH is a rural hospital, we can offer our patients a skilled nursing bed or “swing bed” as a Medicare-funded program. To qualify, a patient must need skilled services such as nursing, physical, occupational or respiratory therapy.
